About Flourish Labs//Peers.net
We’re on a mission to make mental health support accessible to all by empowering people to support each other.
We grow the mental health workforce by training and employing certified peer supporters. We offer mental health support from peers who get you because they share your life experiences and identity, to provide emotional support, teach skills, and share their knowledge and personal experiences with mental health. Our Gen Z and diverse (50% LGBTQ+ and/or BIPOC) supporters are fully trained, certified and supervised by our clinical staff. We developed our proprietary certification training in partnership with peer support experts, young adults and learning design experts.
Peers.net is our tele-health platform where you can book 1-on-1 appointments with a peer supporter of your choice within 48 hours. We co-created peers.net with young adults to build a mental health service that meets their needs.

About this role
As the Care Coordinator, you’ll report directly to the Head of Operations and work closely with the Peer Team. Your role will be critical to supporting clients, families, and providers through the referral and booking process.
You will have responsibility for the following:
- Day to day care coordination
- Managing the care coordination process for clients/ families and providers from start to end. Including:
- Responding and coordinating with providers when referrals are made via all referral channels (email, referral form, phone number).
- Reaching out to clients and/or their families to answer questions and help them book a session
- Following up with families as needed to get initial and follow-up sessions booked.
- Work closely with providers to ensure you have the information you need to coordinate with families and to provide any updates when sessions are booked/ completed.
- Process improvement
- Refine processes to ensure a smooth experience for clients and families
- Share insights and collaborate with internal team to improve processes and help us identify how we can better support our clients and their families
- Outcomes management
- Maintain care coordination data dashboard for internal team to provide insight into referrals
- Join partner meetings to share updates or share more about the care coordination process at Peers.net (e.g. Team overviews and monthly supervision meetings)
- Preparing monthly reports for wrap around partners and other key provider partners
- Ad hoc operations support
- Support on additional internal operations projects as time allows (e.g. provider outreach lists, updating provider BD database, following up with providers where needed)
What we’re looking for
Key qualifications:
- Passion for mental health and using technology to solve big problems
- Bachelor’s degree or equivalent practical experience (working previously as a care coordinator or similar or high-volume practical Peer Support working experience
- Highly organized and detail-oriented, with the ability to manage multiple priorities and deadlines.
- Bias for action, ability to work independently, confidence to ask for help
- Thrive in ambiguity, ability to adapt to and drive changing priorities of an early startup environment
- Creative thinker, willing to try different approaches, learn and iterate quickly
- Outstanding written and verbal communication skills. We are a remote-first company with people in different timezones, so we rely on clear written communication to get things done
- Proficient in G-Suite and project management tools (e.g., Asana, Slack).
