We are hiring a full time Health Coach to join us in our Los Angeles, CA clinic to enhance our ability to be part of our integrated care team and support our patients via a true wraparound care model.
The opportunity:
Health Coaches at Amae are a critical part of our patient’s care journey, supporting their physical health as well as their mental health. Reporting to the Clinic Director, the Health Coach is accountable for assessing clients’ physical health, and developing comprehensive health plans that focus on lifestyle changes across the physical-mental health spectrum - inclusive of diet, exercise, sleep, financial health, relationship health, and treatment compliance. Via individual and group coaching sessions, our goal is to help patients maintain their treatment goals, and give them the tools to reduce stress and improve mindfulness, nutrition and medication adherence and increase physical exercise.

What you’ll work on:
- Patient-Centered Health Coaching
- Utilize evidence-based practices to provide health coaching to patients with severe mental illness in a clinical setting.
- Conduct behavioral assessments to understand patients’ health and behavioral needs and develop tailored health coaching plans.
- Implement coaching plans using behavior change principles to identify and overcome patient barriers.
- Wellness & Physical Activity Facilitation
- Lead daily and weekly group activities that incorporate movement and physical wellness into care plans for our members.
- Organize and lead member outings, including hikes, tours, and sports activities.
- Collaboration & Care Coordination
- Coordinate with psychiatrists, therapists, and support teams to provide feedback on patient goal attainment and clinical issues.
- Support behavioral interventions related to dietitian/nutritionist-led dietary modification plans.
- Monitor and evaluate patient interventions and outcomes, making modifications as needed to support the patient’s care journey.
What you’ll have:
- 2+ years of experience in health education and/or health coaching, preferably in the mental health space
- BA/BS in appropriate field of specialization (examples such as Health Education, Exercise Physiology, Respiratory Therapy or Dietician)
- Certification, advanced certification, and/or license appropriate to field of specialty as required
- Strong interpersonal skills and a demonstrated ability to work as part of a multidisciplinary team
- Solid understanding of basic workplace technology, and an interest in utilizing technology to improve outcomes for patients
