Amae Health is looking for a passionate Clinical Care Coordinator to join us in providing exceptional care for complex adult members. This full-time, in-person position will be based in our Los Angeles clinic, in Westwood.
The Opportunity:
An integral member of the interdisciplinary clinic team, the Clinical Care Coordinator (CCC) functions as a coach and mentor; helping members set goals, develop skills, and create meaningful connections within their community. Instrumental in fostering a sense of belonging, the CCC is responsible for facilitating group activities, including education and awareness initiatives. As part of the broader team, the CCC works collaboratively to ensure coordinated care and improved member outcomes. Additionally, this role is also accountable for completing administrative tasks, such as managing communication and maintaining accurate documentation, while continuing to grow through ongoing training and skill development.
What You Will Work On:
- Member Engagement & Support- Facilitate games, arts & crafts, and recreational activities to enhance member experience at Amae Health. Encourage positive socialization and participation in treatment programs
- Collaborative Care- Work closely with the treatment team which includes psychiatrists, therapists, and nurses to ensure coordinated care and improved member outcomes. Engage in clinical meetings to discuss member progress and new cases
- Treatment Planning & Documentation- Maintain accurate member records and update treatment plans as needed. Assess members’ ongoing needs and connect them with appropriate community resources
- Continuous Learning & Development - Participate in training sessions to enhance mental health treatment skills. Stay informed on best practices through learning opportunities

What You’ll have:
- Enrolled or recently graduated in a degree program related to mental health, such as pre-med, psychology, social work, or nursing
- Interest in mental health and desire to work with members with Severe Mental Illness
- Strong interpersonal skills and ability to work in a team environment
- Detail-oriented and organized
- Comfortable working either one-on-one or in group settings, must possess excellent interpersonal skills and be able to adapt to changing situations
- Willingness to collaborate with others
- Well-organized and some experience planning and designing events and activities
- Basic computer skills are required
- The candidate should possess a holistic perspective of “health.” Clinical Care Coordinators will demonstrate the ability to work effectively in a wide range of settings with people from diverse backgrounds, including members and co-workers

Base compensation for this role is $21.00 per hour.
